Luther Park is seeking an experienced, creative, fun and caring Life Enrichment Assistant (Activities Assistant) for an immediate full-time opening from 9a-5:30p and one weekend a month! The Life Enrichment Assistant supports the overall Activities program for nursing home residents at the Trinity Center at Luther Park Community. Responsibilities include one on one engagement with residents, conducting group activities, assisting residents with meals and other supportive activities as defined in residents' care plans.

A CNA certificate is preferred but not mandatory.

Responsibilities:

·         Organize and conduct at least two group activities per shift

·         Visit and interact with residents one on one and conduct small group activities

·         Maintain accurate documentation of residents’ progress and attendance with programs

·         Complete one on one documentation and charting

·         Engage with families by orientating them to activity supplies and provide feedback on resident response to group and individual activities

·         Collaborate with Trinity Center nursing staff as needed to reduce behaviors and provide one on one interaction

·         Serve plated food at mealtime and assist residents with eating as defined in their care plan

·         Assist residents to and from activities and meals as defined in their care plan

The ADON responsibilities will involve clinical management leadership and direction to nursing personnel through planning, organizing and carrying out nursing services and will orientate all licensed personnel to the facility regardless of shift. The Infection Preventionist responsibilities will be to lead and implement the infection prevention and  control program to control the spread of infections and/or outbreaks by providing education to staff, residents and visitors as needed.  This position will be responsible for instructing and supervising nursing personnel and comply with established policies and procedures. 

RESPONSIBILITIES:

ADON

·         Attend daily clinical meetings, resident care plan meetings and monthly QA meetings

·         Supervise daily nursing operations

·         Evaluate individual care plans through resident care conferences 

·         Apply conflict resolution techniques working with staff conflict or family inquiries and ensure follow-up has been conducted 

·         Keep the DON informed of resident status changes or other matters related to resident care or staff concerns

·         Coordinate admissions/discharges and ensure all aspects of admissions and discharges have been completed

·         Monitor that physician/family are informed of resident status change

·         Monitor physician orders 

·         Monitor monthly medication review and follow-up as needed

·         Actively participate in RN/LPN and CNA/CMA meetings

·         Review fall incident reports ensuring appropriate follow-up and interventions are in place and care plans are updated

Summary of Duties
The Manager of Health Services provides supervision and administration of quality nursing care for general assisted living and memory care residents. The Manager of Health Services will be knowledgeable of all state and federal laws and regulations applicable to assisted living. The individual works with the Director of Assisted Living to provide a physical, social and psychological environment conducive to maintaining the highest level of independence and autonomy for all residents of the program.

Essential Functions

• Collaborate with the Human Resources department to recruit, hire, supervise, coach, evaluate and as needed terminate clinical staff who provide health care and related services to residents.
• Collaborate with the Administrative Office Coordinator in creating a monthly schedule that maintains adequate staffing 24/7 and meets the needs of the residents.
• Conduct pre-admission and change of condition resident assessments. Collaborate with the Assistant Manager of Health Services to conduct 30 day, 90 day and annual resident assessments.
• Develop initial service plans for residents and update plans as needed or as required.
• Complete delegations for clinical staff including CNAs, CMAs, LPNs and RNs.
• Share on-call responsibility.
• Collaborate with physicians, therapists, pharmacies, hospice, home care agencies, wellness providers, etc. in coordinating residents' health care needs.
• Review incident reports to identify concerns with residents' safety, track findings and implement changes as needed to address root causes.
• Communicate with family members regarding the health condition and progress of the resident including in-person and telephone discussions when appropriate.
• Collaborate with the Director of Assisted Living to plan and conduct in-service education programs for clinical staff and maintain record of programs and attendance.
• Ensure the facility operates in compliance with all local, state and federal regulations.
• Provide clinical leadership and direction for the biennial Department of Inspections and Appeals survey.
• Develop, revise, distribute and enforce policies and procedures related to health services.
• Prepare and submit records and documentation as required by internal policies, state and federal rules, regulations and laws for assisted living facilities.
• Participate in continuing education to maintain credentials and increase expertise and knowledge for assisted living management.

• Keep DON/ADON informed of resident status and other pertinent matters
• Admit and orientate new resident and involves family with orientation
• Check physician orders, transfer forms and pre-admission information
• Review resident's medical and social history
• Complete admission and documentation forms
• Ensure resident's supplies and medications are ordered and equipment is working properly
• Maintain proper documentation of facility records: narcotic records, medical chart forms, weekly skin assessment
• Maintain daily, weekly or monthly weight and vital signs and informs physician, DON, ADON and On Call Manager of any changes
• Complete incident reports, investigation reports and notify physician, family members, DON/ADON and On Call Manager
• Discharge residents to community or other facilities, transfers residents to hospital, completes required forms and documentation, ensures resident is escorted to transportation and contact family prior to any transfers to hospital

REQUIREMENTS

Graduate of an approved school of nursing for RN/LPN
Current nursing license in good standing in the state of Iowa

Current CPR certification preferred or obtained within 30 days of hire

One year of experience in a geriatric setting preferred

Knowledgeable with current changes in nursing practice
